Cancellation Policy

Youth Basketball Training Waiver I, ______________________________________(Name of parent/guardian), understand that, for ___________________________(Name of youth participant), participation in, equipment used by, and the movements involved in basketball training sessions carries with it a risk of injury. I hereby waive and release Jeffery Johnson & Team, LLC, its instructors, employees, and representatives from any and all liability for any injuries or illness incurred while participating in sports training sessions. Jeffery Johnson & Team, LLC, Jeffery Johnson and employees/staff associated with Jeffery Johnson & Team, LLC will not be held liable for any medical expenses incurred while my child is in the Crowns Basketball Academy. In lieu of a medical certificate signed by a medical physician, I have no knowledge of any physical or mental impairment that would be affected by my child’s participation in the training/camp/clinic at Jeffery Johnson & Team, LLC Before engaging in any physical fitness program, you should consult with your personal physician and advise them of the nature of the program. Training/camp/clinic participants agree that all exercises and/or courses are undertaken at his/her own risk. Training/camps/clinics participant understands that there is a risk of personal injury involved in the course of instruction and with his/her knowledge agrees to hold harmless Jeffery Johnson & Team, LLC, Jeffery Johnson, its instructors, employees and representatives from all losses caused by accident or injury to the training/camp/clinic participant, or to a third person, who may be a camper/visitor/guest of the session, in the event that either the camper/visitor/guest or said third person is injured in any way during the performance and execution of exercises, I agree and understand that Jeffery Johnson & Team, LLC, its instructors, employees, or agents shall not be held liable or responsible for personal injuries or damaged or stolen articles inside or outside of the training/camp/clinic.
